These plans are usually used for group coverages and
generally are a higher level of benefits. They offer
coverage on a co-pay basis only and have NO deductibles. This type of program requires you to select a primary
care physician who will provide a "referral" to a specialist
if necessary. These plans require you to only utilize
providers within the approved network of physicians.
